Community is Key...

...in all that Communities & Physicians Together does. The community provides a rich, non-traditional classroom outside the walls of the hospital and clinics necessary for residents to get the best training possible. The partnership formed by community members and UC Davis ensures that the relationship between future and practicing physicians, and the communities in which they practice, will be better suited to unite around important health issues in a holistic way.

- Children First - Flats Network serves the Alkalai Flat neighborhood of downtown Sacramento. 
 
CFFN works to bridge and connect agency, institution and association resources with individuals to
  improve the quality of life for all children and families in their neighborhood

- Cordova Community Collaborative is a group in the Rancho Cordova community of eastern
  Sacramento County.  CCC believes "all children should grow up in a family and community that
  provides for their health and well being. We are committed to strengthening families and linking them
 
to community resources that enhance self-reliance and promote community involvement."

Harmony Health Family Resource Center assists families in the Linda community of Yuba County
  with referrals, in-home case management, counseling, parent workshops and support groups,
  women's support groups, youth groups and activities, and resource information.

- Mutual Assistance Network of Del Paso Heights "is a non-profit community development
  corporation that utilizes the skills and talents of the residents of the Del Paso Heights neighborhood
  to 
strengthen their community."

- River Delta Healthy Start serves the Courtland and Walnut Grove communities of rural south
  Sacramento county and offers support to rural and migrant farm worker families in the Sacramento
  River Delta area.

River Oak Family Resource Center provides families in the Oak Park community with
  free, voluntary family support and home visitation services.

- Tahoe/Colonial Collaborative is a small, grassroots community organization situated in the
  southeastern section of Sacramento, that works to connect families to each other and to resources
  for improved health, safety and overall wellbeing.


- Valley Hi Youth & Adult Coalition exists to create opportunities for Valley Hi children, youth and families to have successful experiences in school, community, personal positive growth and development. We value youth adult partnerships, fun and collaborative leadership.
